MYDC After-school Program registration is by APPOINTMENT only.

The After-school Program will start on Monday, September 12, 2016. To make an appointment for registration, please call 412.322.0585.

  • The fee is $195 for the 1st child and $40 for each additional child.
  • Students in 6th grade & up are $20.
  • Half of the total registration fee must be paid before students can attend the After-school Program. The remaining balance is due by November 7, 2016.

In addition to basic demographic information, Parents MUST bring the following info…
  1. Medical information (including insurance company & policy number)
  2. Name of Medications that child(ren) is presently using
  3. Any dietary restrictions, allergies, and medical conditions
  4. Name, address, & phone number of the child(ren)'s doctor/primary care physician
  5. Name, address, & phone number of 2-3 emergency contact persons
  6. Social security number for both child(ren) & parent
  7. Up-to-date shot records for every child
  8. Copy of a completed physical for all new students, Kindergartens, and 6th graders
